I'm applying for Polish citizenship and would like to know what some documents mean?

Hello, I'm currently trying to apply for polish citizenship, but I'm having a hard time trying to understand some of the documents that are required. (2)
Wyciąg z Księgi Ludności
Poświadczenie Obywatelstwa Polskiego
If you speak Polish, can you help?! Are those documents, or just data? Thank you.

Both are documents.
"Wyciag" means "pulling data out of" or copying data out of some data collection.
Ksiega Ludnosci would be a database of people living in an area, for example administration of municipality might have such a database. Ksiega means a book documenting something of a legal nature but likely the records may be computerized.
Poswiadczenie means "confirmation of" .. in this case Polish Citizenship. It sounds like this one may be achieved with a passport, an ID card, or something of the sort. Its also possible that instead of using notarized copies of those documents there may be a standard form that is issued to confirm citizenship – but I would have no idea what body would issue those and if that is done.
If i would be in your shoes i would go to eng-pol Translator because it is too hard for me to translate it but i will try my best:
statement from tome of population
certify of polish citizenship – i think it just means to get polish citizenship
What is the sense of applying for a Polish passport if you are not able to understand the language at all ? My strong suggestion is hiring a professional translator because you can sue him if the translation is wrong unlike the people here on this board.
