I am planning on getting one of the two tattooed on my back and I want to get it perfect.

Please notice: "diacritic" is a mark such as an accent, placed over, under or through a letter in some languages, to show that the letter should be pronounced differently from the same letter without a mark.
Since you are going to tatoo a phrase, you don`t need diacrtitics for it. You need them to know how to pronounce the words.
You tatoo "Na Zdrowie" or "Na Zdravi" and that`s it.
But you pronounce as:
nA zdrOvie / in Polish/ and
nA zdrAvi /in Chech/. The stress is on capital letters.
P.S. Because you like this phrase I will enrich your vocabulary with two more languages:
In Bulgarian you say as: nA zdrAve
In Turkish: afiEt olsUn.
P.S.2 Please consider the opinion of Sander L for the Chech diactirics.
Souri is NOT correct… Writing " na zdravĂ " without the accent over the " i " is incorrect. I don’t know the Polish, but the Czech form definitely has an accent over the " i "
na zdravĂ
